What is the Thai DLT Medical Certificate?

Under the Motor Vehicle Act B.E. 2522 (1979), every applicant for a new Thai driving license or 5-year renewal must present an official medical certificate (ใบรับรองแพทย์).

The Department of Land Transport (DLT) requires all drivers, Thai citizens and foreigners alike, to certify their physical fitness before receiving a 2-Year Temporary or 5-Year Permanent Driving License. The certificate proves that you do not suffer from severe chronic conditions that could impair safe driving on Thailand's roads.

In 2021, the Department of Land Transport standardized the medical form into a bilingual two-part official document: Part 1 is a self-declaration of your medical history signed by you, and Part 2 is the formal clinical evaluation signed and stamped by a licensed medical practitioner registered with the Medical Council of Thailand.

Two DLT Rules to Remember

1. The 30-Day Expiration Clock: Your medical certificate is valid for exactly 30 days from the date of issue. If your DLT appointment is delayed past day 30, officers will reject the paperwork immediately. We time your clinic visit so the certificate is still valid on your DLT date.

2. Applying for Both Car & Motorcycle? You Need 2 Originals: The DLT maintains separate physical archives for motorcycle and car licenses. Photocopies of medical certificates are strictly rejected. When you book our Combo Package, we arrange two original certificates with live doctor stamps so both files are complete.

The 5 Mandatory Health Conditions Checked

The official Department of Land Transport evaluation screens specifically for five statutory health conditions that disqualify drivers under Thai law.

1. Contagious Leprosy

Leprosy in an active, contagious stage or causing severe physical deformity that hinders vehicle operation controls.

2. Advanced Tuberculosis

Active pulmonary tuberculosis in a contagious stage that poses a public health risk or causes severe respiratory distress.

3. Substance Addiction

Severe chronic alcoholism or active drug addiction that impairs mental alertness, motor reaction times, or conscious judgment.

4. Chronic Elephantiasis

Elephantiasis in advanced symptomatic stages that physically obstructs normal limb movement or foot pedal control.

5. Severe Neurological Disorders

Uncontrolled epilepsy, active seizures within the past 12 months, or severe psychiatric illnesses that endanger safe driving.

Note for Healthy Expats: For 99% of foreign applicants, the examination is completely routine and takes less than 10 minutes. The doctor checks your blood pressure, listens to your heart and lungs, checks basic color vision, and signs the clearance form.

Frequently Asked Questions: DLT Medical Certificate

Clear answers to the most common questions expats ask about the health check in Phuket.

The medical certificate is strictly valid for exactly 30 days from the date it is signed by the examining doctor. If more than 30 days elapse before your DLT appointment date, you must obtain a new certificate. For this reason, we schedule your clinic visit just days before your DLT appointment.

Yes, absolutely. The Department of Land Transport in Thailand files car and motorcycle licenses in completely separate government archives. The DLT requires two separate original medical certificates with live doctor signatures and clinic stamps. A photocopy will not be accepted. We arrange two originals simultaneously at our partner clinic.

No blood or urine tests are required for private car (Class 1) or motorcycle (Class 2) driving licenses. The examination is completely non-invasive and consists solely of taking your vitals (blood pressure and heart rate), a simple physical range-of-motion test, and an eye chart check.

No. The DLT will strictly reject any foreign medical certificate. The document must be issued on the official Thai Medical Council standardized form and signed by a licensed doctor physically practicing in Thailand with an active medical license number.

You only need to bring your original passport (with your current valid Thai entry stamp or visa). The clinic will transcribe your full legal name, nationality, and passport number directly onto the official certificate.

Yes. All our packages (Motorcycle, Car, and Combo) include complete clinic coordination, appointment scheduling, and your official doctor's certificates. You don't have to worry about finding a clinic or dealing with language barriers.
